This page is a template made available to our Clients, to help them inform their own visitors as required by Articles 12 and 13 of the GDPR. You (the Client) remain the data controller of your forms: adapt these texts to your situation, complete the placeholders in brackets, and have them reviewed where needed. 1st Minute Lead acts as your processor under the DPA.
1. Short notice — to display under the form
The information in this form is processed by [Company name] to call you back as quickly as possible about your request. It is transmitted to our call-back tool, 1st Minute Lead (Reach Technologies SAS), which processes it on our behalf and hosts it in France. To exercise your rights or learn more: [link to your privacy policy].

2. Full section — to add to your privacy policy
Call-back of your contact requests
When you submit a contact or quote form on our website, we use the 1st Minute Lead service (Reach Technologies SAS, France) to alert our advisors immediately and call you back as quickly as possible.
Data processed: the fields of the form you submit (in particular name, phone, email, message), the page from which you sent it, your arrival source on our website (referrer and campaign parameters), and a journal of your visit on our website only (pages viewed, clicks, searches on this site), which gives our advisor the context of your request. This journal is kept in your browser's session storage and erased when you close the tab; no cookie is placed. [If Scoring enabled:] This data is also analyzed automatically to estimate the nature and urgency of your request; this score merely helps our advisors prioritize and every decision is made by a human.
After your submission: a window may offer you an immediate call-back, a call-back slot of your choice, or a conversation directly in your browser (with your microphone, and your camera only if you enable it — the conversation is neither recorded nor transcribed).
Legal basis: performance of pre-contractual measures taken at your request (Article 6(1)(b) GDPR) [adjust if you rely on consent or legitimate interest].
Recipients: our advisors, and Reach Technologies SAS as processor (hosting in France; list of its providers: subprocessors page).

Your rights: you may access, rectify, erase your data, object to or restrict its processing, by contacting us at [your contact email]. You may lodge a complaint with the supervisory authority (in France, the CNIL).
3. Checklist for your compliance
- Display the short notice near each monitored form.
- Add the full section to your privacy policy.
- Limit your form fields to what is necessary (data minimization) and never collect passwords, payment data or sensitive data in a monitored form.
- If your consent banner governs measurement/tracking on your website, connect it: the 1st Minute Lead script natively recognizes Cookiebot, CookieYes and Usercentrics, and can be initialized in "denied until consent" mode (see your installation page).
- Define and document your retention period for non-converted leads.
- Organize the handling of rights requests (search, rectification, deletion are available in your 1st Minute Lead interface).
